Import QTI file to new Item Bank doesn't work

I can't import an entire quiz to an Item Bank.  I've tried exporting a classic quiz and uploading the QTI file to a new item bank and it says "Error: there was a problem uploading your file."  Canvas created the QTI file so it has to be the correct format or type.  Can someone please explain to me why this no longer works and how to fix it?  

My only other option is to continue uploading question banks as classic quizzes, migrating them to new quizzes, and adding each question to a question bank manually (which takes forever).

To clarify, here are the steps I've taken following multiple tutorials on the process:

1) create QTI file.  I've done this by creating a plain text document with the correct formatting and uploading it to two different websites who then create a qti file.  I've also tried exporting a classic quiz and using that qti file.

2) create new item bank

3) select "import content"

4) select my file by browsing or drag and drop (tried both)

5) Fail

   2. making sure to use the option that is available for "Convert content to New Quizzes"

I am asking because those are the steps that I went through and everything is working.

I am able to import a quiz with the qti file, but I want to be able to import all questions to an Item Bank.  If i select New Quizzes then I can't add to an Item Bank.  If I select Classic Quizzes and add a new Item Bank, it imports the quiz, but no Item Bank.  I then have to migrate the quiz to a New Quiz and edit each question individually to add it to an Item Bank.  I would like to be able to import all questions from the quiz to a new Item Bank.

I like to create Item Banks for each topic and then select questions at random from each one for assessments.  For example, I might pull 5 questions at random from Item Banks titled 'Vaping', 'Tobacco', and 'Alcohol' for an assessment instead of creating one big quiz with all of the questions.
