Students Unable to Submit Multiple Attempts

Since the release of the the "Build on last attempt" option, there have been many reports of students being unable to submit multiple attempts for New Quizzes when multiple or unlimited attempts are enabled. Students can usually submit 2 or 3 attempts without issue, but when they go to start an additional attempt, they get a message saying they have already used their 1 attempt or they don't have the option to begin a new attempt. It seemed like a browser caching issue at first, but changing browsers/cache clearing doesn't resolve the problem. Occasionally, the issue can be fixed by manually giving a student an additional attempt through Moderate, even if unlimited attempts are already enabled. 

This has been affecting multiple quizzes and different courses. Is anyone else experiencing this issue at their school?
