Generally speaking, a Canvas course shell starts out completely empty, and instructors can create content such as quizzes, assignments, discussions, content pages, etc. and fill them with whatever content they want for the course. By "assignments", are you asking about a quiz/test/exam in your course? If the instructor is creating this from scratch, then yes, it starts out completely blank, and the instructor would have to create the questions for the quiz/test/exam. However, there are other sources that your instructor might use to get quiz/test/exam questions. One source might be questions from a book publisher (questions that came as a supplement to the textbook, for example). Or, another source might be a large repository of resources that instructors have access to (called Canvas Commons) where instructors might share questions that anyone could incorporate into his/her course. But, to get to the root of your question, a quiz/test/exam usually starts out blank/empty, and then an instructor can fill it in with different question types for students to take online.
