Photos uploading on quiz as grey box

Every time I try to insert a photo onto my canvas quiz it shows up as a grey box rather than displaying the photo. I have attached a photo of what it looks like.

Are you dragging and dropping the image to the text box?  Or are you actually uploading the file to your course?  

From your image I see you are in the Rich Content Editor.  Perhaps these guides can help:

Which browser are you using Canvas in to add the image? Does the same thing occur when you try an alternate browser? Also, would you try to clear the cache?
