Hello @wtsoi55 Welcome to the Canvas Community. Thank you for posting your question. Even though the Canvas Community site is really designed for questions about Canvas, I can try and help you a bit with your question. I'll make an assumption that you want to create a Google Form to embed in a Canvas course. There are all kinds of websites and video tutorials that can help with this...but here are a few that I found that might be helpful:

You can then embed your form in a page of content within your Canvas course. See https://community.canvaslms.com/groups/designers/blog/2015/04/23/using-google-docs-for-dynamic-canva... for some examples.
