[ARCHIVED] Adding checkboxes within a page HTML code?

I was wondering if there was a way to create checkboxes within a page, where each bullet point could have its own checkbox that needed to be manually checked off. Does anyone have anything similar or an HTML code to share?

If you would like to have the Community Team restore the original page that @urbansk6 had linked to, you can use the link in @urbansk6's response.  Near the end of the description under the heading "What if I lose access to something I need?", there is a link to "completing our form".  Fill out the form, and hopefully you'll be able to get that page restored so you can see the solution that was offered.

When you click on the link in @urbansk6's above reply for "Checkboxes for Page in HTML editor", you should see this area of the page to request the archived page be revived...

This area of the page is found right above where all the comments start on the page.

I had to do a page search. on the linked page on getting help. Just seems this UI/UX would be a little more user friendly...
