I have never had this issue before so I do not know how to go about fixing it. Over the past couple of years I have created questions in question banks in new quizzes. I then make my quizzes and tests from those banks. Each year when I copy the previous years course the tests and quizzes are set up and ready to go. For the first time just this semester my students cannot see any pictures that were embedded in the questions. It looks like the questions were not copied to the new course, but I am not sure how to fix this. I have shared the banks with the new course, the pictures are not located in my files as I made sure to share those as well. I have even tried remaking the quiz or test and that has not fixed the issue either.
How do I get embedded pictures from questions in questions banks (new quizzes) made in previous years to show up in my current years classes?

We had the same problem at my institution and Instructure support said to change the way we copy the quizzes, by using the Export/Import tools within New Quizzes instead of the Course Content Import tool or the super simple Copy to option next to individual quizzes.

Honestly, I think the Export/Import tools require more steps and are much more difficult to find/explain to instructors, so I'm hoping the problem with embedded images gets fixed in a future release. Being able to use the Copy to function or regular Course Content Import tool is so much easier and these are the steps we've been telling faculty to use for years.

I am sorry you are running into this issue with your images on your questions.
I know that Instructure has listed a known issue listed HERE about images shared to item banks if the original course/item is deleted.
Odd question, but are you using Respondus Lockdown browser on the New Quizzes that this is happening on?
