This is really frustrating because, after the first attempt, students can see their answers and the correct answer. They could then go back, make a second attempt with the answers from the quiz in hand. And, as best I can tell, there is no way to automatically take the first quiz instead of the highest.
Until this is fixed, my work around will be to tell my students that their first quiz will count (I am even putting this in my syllabus) and I will manually go in and change their quiz grade to the first attempt if they take it more than once. (Thank you Kaitlin Seaman for the idea).
I am attaching a screenshot illustrating the problem. Notice that the bar up top (highlighted in yellow) notes that there is only one attempt allowed. However, after taking the first attempt, notice that the retake quiz button is there. That's the problem.

That does sound frustrating. Are you looking for Instructure to implement a change to how the quizzing feature of Studio Quizzes works? If so, you could certainly submit that as an Idea here in the Community so that it could be evaluated. Here are some helpful links for you:

I might have a fix for this based on how you embed video quizzes into your course.
Embedding with the external tool (so gradebook auto-updates):
Submission type is External tool (find Studio, then your video); uncheck the box for Load in a new tab; go back to change the name of the assignment; Allowed attempts is Limited; Number of attempts is 1; Save and publish. When I go look at it under Student View, it does give me the button to retake the quiz, but it doesn't actually let me retake the quiz, it just keeps cycling me to the Get Started button then back to the Retake Quiz button with the video's audio replaying.
Embedding without using the external tool:
Why would you NOT embed using the external tool? For me, the video screen presents very small. I know students can expand to full screen but I'd prefer they not need to.
Before you do the embedding, type a statement such as this "INSTRUCTION: The gradebook will not automatically update once you submit the quiz. The instructor will update the gradebook after the quiz closes." Then go to the Studio button, find your video, Video quiz embed, choose the quiz, Embed. Display grade as percentage (this is easier to manually update the gradebook, unless you want to calculate each student's percentage into a point value to update the gradebook). Submission type: online, text entry. Allowed attempts is Limited, and number of attempts is 1.
