Mastery Grading and Learning Mastery Gradebook Item Alignment

I'm one of the few people doing Learning Mastery having to input my own outcomes, so I don't have help from my district or site, so hoping someone can help me.

I have inputted my own outcomes in my courses (so much work!). I have linked each individual question on an assessment to an outcome. When students get a score for an outcome it says "no alignments." Sometimes it has shown alignments and sometimes it hasn't. I think if I tag the quiz with all the outcomes that the alignments showed up for some outcomes, not sure why it did it for some, but not for others. 

I allow retakes, and I don't want their retakes to lower their scores, as I have told my students they can only help. However, without the alignments listed for the different times they attempted the outcome, it is difficult to tell if their old level was higher. I do not have an option of weighting the highest score, only the latest score which makes it difficult to get an overall level from multiple attempts. I have suggested this option in the request a feature page. 

Am I setting them up incorrectly to get "alignments?" Does it only show alignments for the overall test or should individual test items be showing up as "alignments?" What happens to the overall level of mastery for an outcome when "alignment" might be generated from an overall score on a quiz or test rather than the individual test questions? 

I am using New Quizzes, and I have read a few things where that might be an issue, but it also could be the way I'm inputting outcomes or setting up the tests.

Any help is appreciated.
