Canvas automatically reverting quiz grade posting policies (muting)?

Has anyone experienced Canvas automatically reverting quiz grade posting policies (from automatic to manual) even if we have it set to automatic for all quizzes by default?

So once the students have attempted a quiz, the quiz appears as "muted", whereas normally they would be able to view their grade straight away?

What could cause the switch from automatic to manual? Could there be an error when copying over modules or editing quiz content?

If a quiz has a "manual" posting policy when it is imported, it does not matter what the course-level policy is. A newly created quiz will have the same policy as the course-level policy. If you change the course-level policy at any point, all existing quizzes will then have a posting policy to match. So, if everything is supposed to be "automatic", you can switch to manual and back to automatic and it should capture everything.

If there are any existing submissions under a manual policy, you will have to "Post" before grades will be unmuted, despite changing to "automatic".
