Help tailoring practice quizzes

I'd like students to be able to take practice quizzes in which: 1) if any questions are unanswered when the quiz is submitted, the correct answer will NOT show in the feedback, and 2) questions that a student answers correctly will not appear (or will already be filled in) on subsequent practice runs. I can't find any way to do this, which means that

the practice Q&A can in the first case be circumvented, and in the second will be experienced as tedious punishment rather than reward... Help? 

Other settings that don't show up in the default space? Workarounds? Outside app I should build the practice in instead?

Thank you for getting back to me. However, I think that making such an assumption is misguided. I gave up. I’m probably not the only one.
